The Monday report existed because the systems underneath it did not talk to each other. Somebody exported accounts from the CRM, somebody else pulled the open tickets, and a third person reconciled renewal dates by hand.
It was already out of date in the meeting
By Thursday two tickets had closed and a renewal had moved a fortnight. Nobody went back to correct the sheet, because correcting it meant doing Monday over. So the room kept making decisions from a photograph taken three days earlier.
What took its place
The timeline is the report now. It summarises nothing and it is not especially pretty. It is simply current, because the work writes to it as the work happens. Monday is fifteen minutes long and starts with what changed.
